giving permission. You have the right to
revoke an authorization in writing unless
Hattie Larlham has already taken action
based on the authorization. To revoke an

Uses and disclosures required or permitted
by law
The law permits, and
in some cases requires, Hattie Larlham to
use and/or disclose your protected health

Hattie Larlham may use or disclose your

or tissue donation from you or a transplant

if you are a member of the military for
certain limited purposes or for national
security or intelligence activities.
- Your workers’
compensation claim to workers’ compensation
agencies.
When applicable, Hattie Larlham will comply
with state and federal HIPAA laws regarding
use or disclosure of your protected health

Uses and disclosures for treatment, payment
or health care operations
Hattie Larlham will use
and disclose your protected health information
as needed for treatment, payment and health
care operations purposes. The Hattie Larlham
entities will share protected health information
with each other to carry out treatment,
payment or health care operations activities
of the organized health care arrangement.
Business associates
As part of the services
Hattie Larlham provides, it sometimes seeks
the help of outside persons or businesses.
These outside entities help with treatment;
payment and health care operations activities
like providing legal help, accrediting facilities
or auditing records. To obtain their help,
Hattie Larlham often must give them your
protected health information. Before Hattie
Larlham gives any protected health information
to these outside parties, it requires them

Facility directories
Hattie Larlham does maintain
a directory at some of its facilities. You
have the right, at the time, to register
at the Hattie Larlham Center for Children
with Disabilities to exclude all or part
of your information from a directory and
restrict what part of the information is
given and to whom. Unless you ask to be
excluded or for a restriction at registration,
your name is included in the directory and
your general condition may be disclosed
to those asking for you by name.
Others involved in your care
If you approve, and if
needed, Hattie Larlham may disclose your
protected health information to people (typically
family and friends), who are involved in
your care or in payment of your care. If
you are not available, incapacitated or
have a health emergency, and Hattie Larlham
determines that a limited disclosure may
be in your best interest, Hattie Larlham
may disclose protected health information
with the people involved in your care or
paying for your care without your approval.
In some cases, Hattie Larlham may also disclose
your protected health information to parties
involved in disaster relief to help them
find your family member or other persons
involved in your care or paying for your

Fundraising and marketing

you during its fundraising efforts. You
can ask to opt-out of receiving fundraising

to receive communications about your protected

While personal information
like your name, address, phone number, e-mail
address and ordering history may be retained
for our records, personal financial information
is not retained. At no time will Hattie
Larlham, its agencies or programs like Hattie’s Café & Gifts retain your personal financial

that which is necessary for these aforementioned
companies, organizations, agencies and programs
to responsibly maintain their own financial

As such, the Hattie Larlham Foundation is
a charitable organization that is legally
allowed to raise funds for Hattie Larlham,
any of its companies, organizations, agencies
and programs for the benefit of people with
disabilities. As a constituent of Hattie
Larlham, you have a right to opt out of
these solicitations. Out of respect for
our users Hattie Larlham offers the ability
