Staff Nurse
(RN/LPN)
Reports To:
Director of Nursing
Status: Hourly, non-exempt
status
Job Description:
Monitors, assesses and provides health care
services to residents as outlined in their
Plans of Care and IHP. Provides training
and mentoring of new staff as assigned.
Major Tasks and
Duties:
- Implements the Plan
of Care to deliver preventative and primary
health care services, with a focus on
the desired health outcomes identified
for each individual.
- Works as a team member
under the direction of the Unit Nurse
to ensure a high quality of health services
and to achieve a balanced workload on
each shift.
- Communicates ongoing
health issues and treatment recommendations
to help families make informed decisions
and feel supported.
- Ensures that unit staff
understand the health services necessary
for them to give the best quality of care
to each individual.
- Assists the Direct
Care Staff and Unit Coordinators with
staff issues as they arise; demonstrates
respect and professionalism to all staff
in order to foster a pleasant atmosphere
for the people who live on the unit.
- Assists with training/mentoring
of new nursing staff as assigned. Completes
orientation checklists and communicates
any issues to shift supervisor.
- Demonstrates respect,
trust, and confidentiality regarding residents,
families and co-workers.
- Demonstrates initiative
and good judgment in job related activities,
substitutes for other staff when needed.
Adheres to all policies and procedures
including safety, quality, and performance
of job responsibilities within Hattie
Larlham guidelines.
- Documents / Tapes shift
report per guidelines. Provides accurate
report to supervisor each shift.
- Contacts shift supervisor
on a timely basis for assistance / advice
regarding ill residents.
- Accompanies residents
on medical appointments as needed.
- Responsible for participating
in maintaining a safe workplace and attending
annual mandatory safety training.
- Performs other duties
as assigned or requested.
Qualifications:
Must be a Registered Nurse/Licensed Practical
Nurse with a current license in the State
of Ohio. Must have knowledge of regulatory
standards pertinent to the Foundation. Must
maintain current CPR certification. Must
possess assessment and organizational skills.
Mental Functions:
Must display excellent assessment and organizational
skills. Must be able to communicate effectively
and professionally with residents, staff,
physician, parents, members of the interdisciplinary
team, and appropriate others concerning
medical issues.
Physical Functions:
Has the mental and physical capabilities
to perform the essential functions and responsibilities
of the job description. Must be able to
walk, climb, push, pull, bend, read, carry,
hear and lift up to 60 pounds or more throughout
the day. Must have hand/eye coordination,
be able to assemble, pour and clean items
as needed.
Working Conditions:
Will work according to assigned schedule;
may be asked to work other shifts; weekends;
holidays as requested and to relieve other
nurses incase of illness or emergency.
