of
individuals
and
their
families.
Brief
Description
of
Duties:
Coordinate,
write
and
implement
individual
plans
for
8-10
individuals.
Communicate
on
a
daily
basis
to
Interdisciplinary
Teams.
Provide
total
service
coordination
to
meet
the
needs
of
individuals.
Work
closely
with
nurses
and
the
Assistant
Home
Manager.
Supervise
the
immediate
operations
of
the
facility.
Must
provide
24-hour
on-call
support
for
facility
and
program
services.
Qualifications
Education/Training:
Bachelor’s
degree
in
human
services
field.
Must
have
one
year
experience
with
people
with
disabilities.
Must
meet
federal
requirements
of
QMRP.
Experience:
One
year
experience
in
the
disabilities
field.
Supervisory
experience
is
preferred.
Other
Requirements:
Must
possess
excellent
communication
and
organization
skills.
Must
be
able
to
problem
solve
and
have
the
ability
to
work
independently.
Utilize
all
Microsoft
programs
and
Casetrakker.
Must
have
the
ability
to
complete
things
in
a
timely
manner
and
be
able
to
meet
deadlines.
Must
have
received
no
corrective
action
for
one
year.
